Collaborate with physicians, nurses, and other healthcare professionals as an integral member of the multidisciplinary healthcare team to deliver safe, effective, and patient-centered care.Provide comprehensive clinical pharmacy services across various medical and surgical specialties, ensuring optimal medication management and therapeutic outcomes.Participate in daily multidisciplinary rounds and review patient medication therapies, recommending evidence-based interventions and treatment modifications when appropriate.Serve as a clinical resource and provide expert pharmaceutical advice and drug information to healthcare professionals, patients, and caregivers.Ensure the safe, rational, and cost-effective use of medications in accordance with hospital policies, clinical guidelines, and best practice standards.Support and monitor compliance with the hospital’s Antimicrobial Stewardship Program and antibiotic usage policies.Conduct medication reconciliation, review prescriptions for appropriateness, and identify potential drug-related problems, including interactions, contraindications, and adverse drug reactions.Monitor patients’ therapeutic responses and laboratory results to evaluate medication effectiveness and recommend dosage adjustments or alternative therapies as required.Provide patient counseling and education regarding prescribed medications, including proper administration, expected outcomes, potential side effects, and adherence strategies.Participate in the development, implementation, and review of clinical pharmacy protocols, guidelines, and medication-use policies.Educate and train medical, nursing, and pharmacy staff on medication-related topics, new pharmaceutical products, and evidence-based therapeutic practices.Document clinical interventions and recommendations accurately in patient records and departmental reporting systems.Contribute to quality improvement initiatives, patient safety programs, and medication management projects within the organization.Stay current with advancements in pharmacotherapy, clinical guidelines, and healthcare regulations to support continuous professional development and high standards of clinical practice. Bachelor's Degree in Clinical Pharmacy.Postgraduate degree in Pharm D, American Board, or Master's degree in clinical pharmacyMinimum 2 years of dedicated clinical pharmacy work experience in the ICU.Knowledge of pharmacology and experience with different medicationsExcellent understanding of pharmaceutical therapy and direct patient careSolid knowledge of drug administration and health & safety guidelines
