Duties and Responsibilities: Medication Dispensing and Distribution:Review and verify medication orders to ensure accuracy, appropriateness, and compliance with established protocols.Dispense medications, including intravenous (IV) therapies, chemotherapy, and controlled substances, ensuring correct dosage, preparation, and labeling.Monitor the dispensing process to prevent medication errors and ensure adherence to hospital policies and regulations.Inventory Management and Drug Supply:Manage the hospital's medication inventory, ensuring adequate stock levels, appropriate storage, and the prevention of expired or recalled medications.Handle controlled substances in compliance with legal and hospital regulations.Collaborate with purchasing and procurement teams to maintain the hospital's formulary and address any drug shortages.Collaboration and Communication:Work closely with physicians, nurses, and other healthcare professionals to provide integrated care.Serve as a drug information resource, providing guidance on medication usage, dosing, side effects, and interactions.Communicate with prescribers to clarify orders, recommend alternatives, and adjust therapies as necessary.Education and Training:Provide in-service training for healthcare professionals on new medications, safety protocols, and best practices.Serve as a preceptor for pharmacy students and residents, offering mentorship and pharmaceutical training.Regulatory Compliance and Documentation:Ensure compliance with all hospital policies, Ministry of Health regulations, and accreditation standards.Utilize electronic medical records (EMR) and pharmacy information systems for accurate documentation and reporting Education:Required:Bachelor of Pharmacy from an accredited institution.Preferred:Doctor of Pharmacy (Pharm.D.) or Board Certification in Pharmacotherapy (BCPS) or other relevant clinical certifications.Experience:Required:A minimum of 1-2 years of experience in a hospital pharmacy setting.Knowledge/Skills/Abilities:Strong knowledge of pharmacology, therapeutics, and pharmacokinetics.Familiarity with hospital pharmacy operations, including medication dispensing, inventory management, and regulatory compliance.Understanding of standards set by accreditation bodies such as the Joint Commission or Ministry of Health.Knowledge of drug interactions, adverse drug reactions, and methods for monitoring therapeutic drug levels.Strong communication and interpersonal skills to effectively collaborate with healthcare providers and counsel patients.Proficiency in pharmacy information systems, electronic medical records (EMR), and automated dispensing technologies.Attention to detail in the preparation, dispensing, and documentation of medications.Ability to work efficiently under pressure in a fast-paced hospital environment.Ability to multitask, prioritize, and handle emergency situations related to medication management.Commitment to ethical standards, confidentiality, and professionalism in all aspects of pharmacy practice.
