This role involves coordinating care plans, monitoring patient progress, and serving as a key point of contact between patients, families, and healthcare teams. Daily responsibilities include conducting follow-ups, organizing and documenting case information, and escalating concerns to clinical staff when needed. The coordinator will provide guidance and basic recovery education, help patients access appropriate resources, and support mental and emotional well-being within established protocols. The role also includes collaborating with colleagues to improve workflows, participating in training sessions, and maintaining accurate, confidential records in alignment with organizational policies.Qualifications Strong interpersonal skills and the ability to build trust with patients, families, and healthcare teams.Clear and professional communication skills, both verbal and written, for coordinating care and documenting cases.Experience or training in case management, including organizing, tracking, and following up on patient recovery plans.Awareness of mental health considerations and the ability to recognize and appropriately respond to patient distress.Ability to participate in and deliver training related to home recovery processes, patient education, and internal procedures.Background in healthcare, social work, psychology, or a related field is preferred.Detail-oriented approach with strong organizational and time management skills.Comfort working on-site in a fast-paced environment and collaborating with multidisciplinary teams.Proficiency with basic computer applications and electronic documentation systems.Commitment to patient confidentiality, ethical conduct, and inclusive, non-discriminatory care.
